Abstract

BackgroundAcyclovir (ACV) is a highly effective antiviral agent with a high rate of clinical efficacy. Current treatments for herpes simplex virus (HSV) infections include antiviral agents (e.g., acyclovir and valacyclovir) and oral antivirals (e.g., famciclovir and famciclovir), with high levels of safety and efficacy. To determine the clinical efficacy and safety of ACV in HIV/STD patients, this study was designed to assess the pharmacokinetics and safety profile of acyclovir and valacyclovir. A prospective cohort study with five months of follow-up was conducted among HIV/STD patients with and without sexual activity. Outpatient HIV/STD patients were enrolled into the study at baseline, and they were treated with the oral acyclovir (ACV) or valacyclovir (VZV) and the other two drugs were treated with the combination of acyclovir (ACV + ACV). The pharmacokinetic parameters were assessed at the baseline, and the pharmacokinetic parameters at 5 and 15 months were assessed. Aciclovir was associated with an increased tmax of 0.9 hours. The AUC0–24 of ACV and VZV was increased by an average of 12.5 and 17.3 hours, respectively. The AUC0–48 of ACV and VZV were increased by an average of 12.7 and 19.9 hours, respectively. There was a trend of a decrease in AUC0–24 of acyclovir and VZV when compared to VZV at 5 and 15 months, but no statistically significant difference was observed between the acyclovir group and VZV group at 5 months. Acyclovir and VZV showed a statistically significant reduction in Tmax and AUC0–24 when compared to the VZV at the same timepoints. However, there was no statistically significant difference in the AUC0–24 of acyclovir and VZV when compared to the other drugs.

Design

We conducted a prospective cohort study with five months of follow-up among HIV/STD patients with and without sexual activity. In this cohort study, patients with sexual activity were defined as patients who were HIV-positive and treated with the HIV-specific antiretroviral drugs. Participants were defined as patients with a positive result for the drug in HIV-infected patients, and these patients were not treated with HIV-specific antiretroviral drugs. The study design was approved by the Ethics Committee for Research in Human Subjects (ECHRE) of the University of Lagos, Lagos, Nigeria, and all participants gave their informed consent. The study was conducted in a community setting with an HIV-positive population of adult patients with HIV. We identified patients who were HIV-positive and treated with the HIV-specific antiretroviral drugs and were diagnosed with HIV-positive or HIV-negative. Inclusion criteria for the study included those who were HIV-positive and treated with HIV-specific antiretroviral drugs. Participants were diagnosed with HIV-positive or HIV-negative and treated with HIV-specific antiretroviral drugs. Patients who were treated with HIV-specific antiretroviral drugs were identified by their HIV status and received HIV-specific antiretroviral drugs. We identified patients who were HIV-positive and treated with HIV-specific antiretroviral drugs and were diagnosed with HIV-positive or HIV-negative.
